I wish to enroll in Texas Health Resources payroll deduct program (Odyssey). I understand that I must be present with my badge when I make purchases and that all purchases made will be deducted from my paycheck on a bi-weekly basis. If my employment is terminated all monies due me by THR will be applied to the balance then due and any remainging balance will be paid at the time of termination.

I choose the installment option (Check next to Installment Option):

Installment Option: $400 (four-hundred dollars) per pay period, charge limit*

This charge limit is ONLY available when using the Installment option for purchases at eligible Volunteer/Auxiliary sales (e.g. Jewelry sales) and the THD Gift Shops (Shops at Main and Health Shops at Perot). I have the option to elect for these purchases to be paid over three pay periods. I acknowledge that I will need to select the "installment option" at the time of my purchase. Otherwise the full purchase price will be deducted from one paycheck.

Cancellation*:

I wish to cancel my enrollment in the Texas Health Resources payroll deduct program (Odyssey). I understand that any outstanding installments and/or balance currently due will be deducted from all monies due me by THR. Cancellation will not take effect until all installment payments are cleared through the payroll process.
